Service Description

If you meet eligibility requirements under the Ryan White Part B program, you will receive the services of a medical case manager. The goal of medical case management is to help you access primary medical care and medications, identify and remove barriers to medical care, and ensure adherence to prescribed treatment. First Steps Schedule an appointment with the MCM. The case manager schedules an “intake” meeting with you. During this meeting, the case manager gathers information about your medical and other needs and determines your eligibility for various programs and services, such as AIDS Drug Assistance Program (ADAP Medication Assistance), Ryan White Part B, and others. For this reason, the case manager may request: Your photo identification Your social security number A verification of your HIV diagnosis Your income and insurance information A list of your current medications Your most current lab reports. Step 2: Ongoing case management After learning about your needs and determining your eligibility for services and benefits, the case manager develops a comprehensive plan for your care and support. The case manager also coordinates the services you require and encourages you to take your medications and live a healthy lifestyle. As needed, the case manager re-evaluates and adapts the plan as your needs change. To help you make continual progress, you will meet with your case manager at appropriate time intervals to address any unmet or ongoing needs that you have.
